Work involves implementing the organization's case management program that also includes developing and maintaining short-term contact with clients, client families, and service providers for social and related service needs.
May train others.
Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.
Interviews and screens clients or authorized representatives to gather information to provides ongoing case management and serves as a liaison between clients, client families, and service providers.

Develops a comprehensive service plan based on client needs, monitors service delivery, reassesses appropriateness, and continuation of services at various intervals.
Coordinates service provider and program activities to address the clients' needs.
Provides ongoing case management and serves as a liaison between clients, client families, and service providers.
Identifies problem areas, service gaps and areas that are barriers to services for clients.
Develops and implements services to meet clients' needs.
Communicates with the public in person, by telephone, fax, electronic or regular correspondence.
Maintains case history, progress notes, and prepares reports.
Adheres to applicable federal/state regulations including, but not limited to, laws related to client confidentiality, release of information, and HIPPA requirements.
May assist in preparing and conducting presentations and orientations.
Performs other such related duties as may be assigned.
EXPERIENCE AND EDUCATION:
Graduation from an accredited four-year college or university with major coursework in social work or a related field is generally preferred.
Experience and education may be substituted for one another.
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:
Knowledge of community resources, of case management delivery systems, and of program policies and procedures.
Skill in developing, implementing, and monitoring service and treatment plans.
Ability to assess clients' needs, to coordinate clients' services, to communicate effectively, and to train others.
Skill in the use of a computer and applicable software.
Ability to read, write and speak both English and Spanish.
