Performs routine (journey-level) case management work.
Work involves developing and maintaining long-term contact with clients, client families, and service providers for social and related Benjamin Rose Institute care consultation service needs.
Collaborates with local partners to identify appropriate participants for selected evidence based interventions, utilizes established criteria and to facilitate successful implementation.
May train others.
Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.
Interviews and screens clients or authorized representatives to gather information to assess service needs.
Develops and implements service and intervention training plans to meet clients' needs.
Serves as a guide to the client, coaching the client in addressing critical issues and self-management.
Provides information and guidance to the client and/or family for an effective evidence-based intervention.
Promotes client self-management and empowers client/family to achieve maximum levels of wellness and independence.
Adheres to applicable federal/state regulations including, but not limited to, laws related to client confidentiality, release of information, and HIPPA requirements.
Identifies problem areas, service gaps, and areas that are barriers to services for clients.
Provides information on finding, using, and monitoring services to clients.
Maintains case history and progress records, and prepares reports.
Monitors clients's progress.
Performs transition planning to ensure continuity of care and management of risk factors.
May train others.
Performs other such related duties as may be assigned.
EXPERIENCE AND EDUCATION:
Graduation from an accredited four-year college or university with major coursework in social work or a related field is generally preferred.
Experience and education may be substituted for one another.
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:
Knowledge of community resources, of case management delivery systems, and of program policies and procedures.
Skill in developing, implementing, and monitoring service and treatment plans.
Ability to assess clients' needs, to coordinate clients' services, to communicate effectively, and to train others.
Skill in the use of a computer and applicable software.
Ability to read, write and speak both English and Spanish.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
