The hiring company is currently looking for a nurse manager to perform as a member of the mental health primary care integration team (PCMHI). This position will communicate with the mental health leadership to discuss process changes, coordinate care and service and future resource planning. Submit completed staff proficiencies in a timely manner. S/he will monitor performance measures for mental health programs to which nursing staff area assigned, ensuring compliance with expected program goals/metrics, and developing action plans, as needed. Triage and applying a collaborative team approach in identifying, analyzing, and resolving patient care problems. Reviewing options with patients and families, including costs, alternatives, risks/benefits, and services. The incumbent will monitor daily environment of care and report needs accordingly. Participate in/facilitate appropriate nursing committee/council meetings. Conduct and document monthly nursing staff meetings and actively participate in mental health staff meeting. Provide follow-up of patient progress with reassessment and collaboration with the primary care provider and supervising psychiatrist. The applicant will coordinate mental health primary care integration management at clinical sites and will provide direct patient care in the absence of regular scheduled staff in the company area.
Requirements
The candidate with 2 to 3 years of mental health experience and 2 to 3 years of supervisory experience preferred. This job requires excellent communication skills and excellent knowledge using Telemental health technologies.
